Admissions
Please visit our campus to receive the Application Packet and take a tour of our facilities.
The following forms & documents must be submitted in order to register/enroll your child:
- Application Form
- Transcripts from previous school (Notarized English translation is required)
- Photocopy of passport and entry visa (if foreign student)
- Authenticated or notarized birth certificate (the school will make a copy)
- Photographs (four 2”x2”)
- Medical Form, including immunization history (to be signed by a doctor)
- Book and Uniform Forms
- Application fee of $100 (non-refundable)
- Transport, Dorm, Scholarship Forms (If applicable)
Withdrawal Procedure
Students wanting to permanently withdraw from school must follow these steps:
- Complete the Withdrawal Form (available with the Receptionist)
- Complete the Transcript Request Form (available with the Receptionist)
- Return textbooks to the Bookstore
- Return locker key (MS-HS) to the receptionist
- Settle all accounts with the Business Office
- Sign the Clearance Form (provided by the Receptionist)
NOTE:
- Documents from the student’s file, transcripts, and Certification Letter will not be released until 2 business after the Clearance Form is signed (the Certification Letter states the student’s enrollment dates and last grade level attended at the school)
- Refunds are issued 7 business days after the Clearance Form is signed
Requesting Transcripts
Students can request one free transcript each academic year. Additional transcripts cost P150 per copy. Please use the Transcript Request Form (available on the website and with the Receptionist). Transcripts will reflect the student’s last three years of academic performance at the school. Please allow 2 business days for preparation of the transcript.
